Prestige Inhome Care is a private provider of high quality care services. With increased demand for services in the Bayside and Inner City suburbs we are now looking for a Case Manager to join our of committed care staff on a permanent basis.
Based out of our Cheltenham office you will:
- Enjoy the support of your colleagues including experienced Case Managers, Nurses and our Client Services team
- Work full time hours Mon-Fri (Part time work, 4 days per week or altered school hours could be explored – please discuss your preference with us)
- Use your exceptional customer service, promotional and case management skills to earn the trust of your current and potential clients
- Know that through your work you are making a difference in your community
This role focuses on the onboarding and ongoing case management of clients who utilise Private and/or Government funding to enable them to remain safely and independently in their home. Central to this role is your ability to build a strong professional and compassionate relationship with your clients and their families, your ability to advocate for their care needs and collaborate with our care teams to deliver services to them in their homes. If you have experience in assessment, case management and coordinating service delivery for older people living at home, and are looking for a dynamic and welcoming workplace, this may be the right role for you.
To be successful, you will need to demonstrate excellent written and verbal communication, time management skills, strong computer literacy and a passion for making a difference in your community. Having with a nursing or allied health background is highly advantageous, however not essential.
