Your Case Manager will be your dedicated contact, who will ensure that your needs are being met and will continue to refine your services in response to any changing needs. Case Manager services offered in our 24-hour program are varied and provide family with peace of mind that their loved one’s needs are being considered from all aspects.
Your Case Manager will conduct regular home visits, health assessments, care plan reviews and team meetings. They are able to work alongside any medical stakeholders and to arrange any allied health assistance or home equipment to ensure the care and your home is all set up for 24-hour care.Personal Care
