As a Case Manager you coordinate services for the client, using the care plan. You also give consideration to the things that are important to them.”
Being a trusted source of information is also a key part of the role. “I advocate for my client’s care needs and help them understand the health care system. As we all know, it can be very hard to navigate and know everything that happens within aged care and allied health services. As a Case Manager you are there to assist your clients and make appropriate referrals for them,” says Melody.
