- Home Equipment Support: Our team can organise hospital beds, pressure mattresses and other equipment to improve comfort, along with practical guidance for safe, effective use.
- Shared Care Facilitation: Support during critical moments and extra care when specialist providers are limited, we offer the clinical knowledge to coordinate with GPs, community teams, and pharmacies. We’ll assist with hospital discharges, medication monitoring and essential supplies.

- Dedicated Clinical Consultant: Your dedicated clinical consultant will work together with your family, care team and health workers, and regularly assess and respond to any changing needs.

- Bereavement Support: We can support you with after-death care, funeral home liaison, equipment pack-up, and compassionate guidance.

Funding Options
End of Life Pathway & Support at Home
The End-of-Life Pathway is a short-term pathway under the Support at Home program. It is designed for older people with 3 months or less to live who wish to remain at home.
The pathway can provide up to $25,000 in funding over a 12-week period. If a participant still needs services after 12 weeks, they may continue using the budget up to 16 weeks if funding is still available. Alternatively, you may still access unspent Support at Home or Home Care Package funds. We can assist you to request an urgent Support plan review.

Privately Funded Palliative Care
If you require care more urgently or extra hours to supplement your government funded palliative care you may opt to pay privately for in-home care.
You may simply choose to fund your in-home care palliative care privately in order to have full control and choice over the services provided, the level of care and how quickly your services are set up.
