Your Prestige Case Manager will work with your loved one to ensure that their individual needs, goals and interests are met through the development of a comprehensive care plan.

With the support of the Client Services Team, your Case Manager works to ensure you and your loved one are supported in scheduling needs and requests, and any additional requirements. Your Case Manager is able to work alongside any medical stakeholders and arrange any allied health assistance or home equipment to ensure the care and your home is all set up for your individual care needs.

Our goal is to relieve our clients from the pressure of creating their own support plan, as well as, supporting those who may not have family readily available.
