Support at Home offers three short-term options,
Each designed for a specific life moment:

1. Restorative Care – Regaining Independence

Imagine recovering from hip surgery. You’re eager to get back to living life as it was. Restorative Care provides funding for up to 16 weeks of tailored support – for example physiotherapy, occupational therapy, and personal care – to help you rebuild strength and confidence. Prestige Inhome Care can coordinate these services so recovery happens safely at home.

2. Assistive Technology & Home Modifications – Making Home Safe

Simple changes like grab rails, better lighting, or mobility aids can transform a home into a safer space, giving confidence to stay independent. Our team at Prestige can help you access this funding and arrange trusted professionals to make these improvements quickly.

3. End-of-Life Pathway – Comfort and Dignity

The End-of-Life Pathway provided specialised palliative care, emotional support, and equipment to make the home environment comfortable. Prestige’s compassionate team ensures families can focus on what matters most – being together.

Why These Pathways Matter

Short-term pathways recognise the reality of unexpected challenges and are designed as a preventative measure to maintain independence, wellbeing. They aim to reduce repeat hospital stays by helping people re-mobilise, stay active and be in their preferred environment.

  • Restorative Care – helps reduce hospital readmissions and delays entry into residential care by supporting recovery and re-mobilisation after illness or injury.
  • Home Modifications – adapt the home environment, making them safer, preventing accidents, improving stability and increasing independence.
  • Assistive Technology – provide aids that builds confidence and capability, enabling people to stay active and maintain independence at home.
  • End-of-life pathway- recognises peoples wish to spend their final time at home, surrounded by the people and things they love. It addresses their pain management, palliative care supports and the needs of their primary carers.

Support at Home provides additional funding for these pathways, ensuring older Australians can improve their quality of life while minimising health risks, remaining connected to their everyday life, in their own home. 


There’s no place like home…

Can You Access Short-Term Pathways While on Ongoing Support at Home? 

Yes. Participants already receiving or approved for ongoing Support at Home services and quarterly budget funding can also be approved for short-term pathways if assessed as necessary. These are additional funds, not taken from your ongoing budget.  

The exception is End-of-Life pathway. On-going Support at Home funding does not continue in tandem with this pathway.

Funding Details: 

  • Restorative Care: approx. $6,000 for 16 weeks (may increase to $12,000). 
  • End-of-Life Pathway: approx. $25,000 for 12 weeks. 
  • AT-HM: separate funding tiers for assistive tech and home modifications.

Is there another Waitlist? 

As this funding is related to unexpected challenges and high-needs support, the applicants can be triaged and referred to a high priority assessment process, allowing them to access the funds and begin services as soon as possible.
