Post-hospital care involves assessment, planning, intervention, and outcome evaluation after an in-hospital treatment.

Falls are a risk following surgery or treatment, and can hinder the healing process. Overnight or 24–hour care in these instances can often give family members peace of mind that their loved ones will be assisted during the night if required. Our aim is to assess the changing needs of our clients and tailor care accordingly.
Our Post-Hospital Support Services
Our post-surgery carer and/or nurse will provide the following personalised services to match your post-hospital requirements until your health and independence is restored:
Showering, dressing, toileting and hygiene
Comprehensive or flexible in-home nursing ranging from 1-2 hours through to round-the-clock nursing support.
Transport to medical appointments or for social outings or grocery shopping.
Paying bills, meal planning/preparation, and prompting medications: where a registered nurse or medication-assist trained personal care worker is on shift, medication can be administered.
Support after a Day Procedure or Cosmetic Surgery Process?
We can tailor our post-hospital care to suit whatever your circumstances may require. We offer discreet, comfortable transportation to and from any day procedures, can collect prescriptions, manage wound care and provide personal care assistance in the comfort of your own home or in a luxury hotel apartment.

You will be connected with one of our Client Service Coordinators who will look after all your rostering requirements and will be responsible for matching your loved one’s care needs with the most suitable nurses and/or carers
Your assigned Prestige Case Manager will establish a unique and flexible care program that is personalised to meet your needs.
