What is the End-of-Life (EOL) Pathway?
The End-of-Life Pathway is a short-term pathway under the Support at Home program, designed for older people with 3 months or less to live who wish to remain at home.
The End-of-Life Pathway works alongside any government-funded palliative care you or your loved one may already receive.
Prestige Inhome Care offers a comprehensive range of end-of-life care services from personal care and in-home nursing to transport and domestic assistance, which can be funded via the End-of-Life Pathway.
We work alongside GPs, hospitals, pharmacies, and specialist palliative care providers to ensure end of life care is seamless and families are fully supported throughout all the stages.

How much funding is available?
Under the End-Of-Life Pathway funding of up to $25,000 is available over 12 weeks.
If an older person has funding left, they can continue using it for an additional 4 weeks, providing them 16 weeks of services in total.
- Depending on your needs you may also be able to access equipment to help with mobility such as a wheelchair or walking frame and equipment and equipment to help you live at home such as adding handrails to your bath or shower. Such support is available through the Assistive Technology and Home Modifications (AT-HM) scheme, under the Support at Home program.
Who is eligible for the End-of-Life Pathway?
A person may be eligible for the End-of-Life Pathway if they meet all of the following criteria below or if they are homeless or at risk of homelessness.
65 years of age/older
Life Expectancy
If a doctor or nurse has estimated their life expectancy as 3 months or less.
AKPS
If they scored 40 or lower on the Australian-modified Karnofsky Performance Status – mobility/frailty indicator.
How can the End-of-Life Pathway funding be utilised?
If a service is on the national Support at Home Services list it can be accessed via the End-of-Life Pathway funding. Here are the 3 categories of support services which can be accessed via the pathway:

How much does the EOL Pathway cost?
The End-of-Life Pathway is part of the Support program, therefore you may need to make a contribution towards the cost of any independence and everyday living services that you receive.
You would not need to contribute towards the cost of any clinical supports as these are fully funded by the government under the program.

What if you need services beyond the EOL Pathway?
If you or your loved one needs services beyond what is covered by the EOL funding or the 12-16 week period, you can request an urgent Support Plan review from My Aged Care so that you can continue to receive services under an ongoing Support at Home classification. If you or your loved one requires services more urgently and wish to stay home you can access private in-home care services, and these can complement specialised palliative care services.
