What Services are Included in Home Care Packages?

Home Care Packages are designed to provide a range of support services that help you or your loved one remain independent and comfortable at home. The services we offer include, but are not limited to:

  • Personal Care – Assistance with showering, bathing, dressing, and grooming, as well as help with grocery shopping and other daily tasks.
  • Nursing – Flexible in-home nursing for various needs, ranging from short visits to around-the-clock care and support.
  • Case Management – Our experienced Case Managers assess, plan, and coordinate services to ensure your care needs are met.
  • Domestic Support – Help with household tasks, including meal preparation, cleaning, and more.

You may be eligible for up to $61,440 each year from the Australian Government to subsidise home care services.

How to Apply for a Home Care Package

Applying for a Home Care Package involves a few straightforward steps to help you access the support you need. Here’s a quick overview of the process:

  • Step 1: Contact Prestige Inhome Care for Guidance
    Call us to discuss your situation, and we’ll help you understand the application process. Our team can answer any questions and guide you through each step.
  • Step 2: Apply for an Aged Care Assessment
    Schedule an assessment through My Aged Care to determine your eligibility. This assessment will identify the appropriate level of care based on your needs.
  • Step 3: Receive Your Eligibility Letter
    Once assessed, you’ll receive a letter confirming your Home Care Package level and placement on the waiting list.
  • Step 4: Work with a Prestige Case Manager
    Once your package is approved, our case manager will help you create a tailored care plan to match your requirements.

While you wait for your Home Care Package to be approved, you can start with private self-funded care. This allows you to access the support you need immediately, and once your package is allocated, we can seamlessly transfer the funding to cover your ongoing care.

Only pay for the services you need

  • Figures above are effective from 1 July 2024. Please note, they will change from 1 January 2025.
  • Package Management includes the ongoing administration and financial management of your Home Care Package, including monthly statements, the purchase, co-ordination and organising of this party services (outside of clinical decisions) including compliance, and quality assurance, along with infection control policies and procedures and the coordination of supports.
  • Care Management this includes the Initial Assessment and complete establishment of your Care Plan and ongoing care needs, care co-ordination, as well as ensuring services are aligned with care goals.
  • These figures have been rounded up except the Income Tested Fee.
  • The Package Management Fee and Care Management Fee are based on a percentage of the available government subsidies and the fee may increase in line with subsidy increases. If the subsidy changes over time, so too may the package management fee and care management fee.
  • Third Party providers: Prestige will provide Third Party Services from our Preferred Providers. Where Prestige does not have a preferred provider that is able to service your area, we will waive any additional set up costs.
  • Hours are approximate, and are based on a 2-hour minimum weekday, personal care rate and will vary based on the time of day, day of the week, services required and/or public holidays.

What if I am waiting for my HCP to be allocated?

Many of our clients choose to begin with private self-funded care whilst they wait for the Home Care Package to be allocated.

Once your package is approved, your current care roster and care team remain in place and we simply transfer the funding across to your Home Care Package and assist with all of the paperwork.

Starting with private care allows you to access the support you need immediately, ensuring there is no gap in services while waiting for government funding to become available.

How Can I Apply For A Home Care Package?

Follow these simple steps to set up your home care package: Give Prestige Inhome Care a call on 1300 10 30 10 and we’ll guide you through the process

  1. Apply for an assessment by visiting My Aged Care and follow the prompts, or call 1800 200 422
  2. You will next receive a letter from My Aged Care which states what level of home care you are eligible for and your priority package level. This letter confirms you are now on the government managed national waiting list
  3. You will receive a second letter indicating when your Home Care Package is 90 days away. Once you receive this letter it’s best to complete the Aged Care Fees Income Assessment form.
  4. Once approved for a Government Home Care Package, talk with a Prestige Inhome Care Case Manager. Let us work with you to develop a budget and care plan that best suits your needs.

Are there any entry/exit fees with a Home Care Package?

No, Prestige Inhome Care does not charge any entry or exit fees for Home Care Packages, allowing you flexibility and peace of mind when starting or ending your care.

Why do I need to pay Care and Package Management Fees?

Care and Package Management Fees cover the cost of coordinating and managing your care, ensuring you receive high-quality services tailored to your needs. These fees include administration, care planning, and ongoing support.

What Home Care services does Prestige provide?

How much does the Home Care Package cost?

How much control do I have over how my Home Care Package is used?
