What To Consider When Bringing Your Loved One Home From Hospital.

When your loved one is in hospital following an injury or surgical procedure, with the right support in place, you may be able to bring them home to recover instead of staying in the hospital or moving to a respite centre. However, for a safe and optimal recovery, it’s important to be prepared by planning for their return home before their discharge date arrives.

When planning to bring your loved one home consider:

Their discharge plan:

This will be created by the hospital, typically in conjunction with your loved one and your family and will outline things like follow-up tests and appointments, medication, equipment and/or rehabilitation your loved one may need.

Who will care for your loved one during their recovery:

While your loved one is recovering, they’ll need assistance with their daily living needs. This may include things like help with showering and dressing. Or assistance managing their medication or any wound care. You may also need to consider housekeeping, like shopping for groceries, cooking meals, cleaning and laundry.

If any home modifications are required:

If your loved one needs to use a walker, wheelchair, or even a cane during their recovery, you may need to make modifications to their/your home to accommodate for and support the use of these. This could include barrier free entryways, bath seats for showering or grab rails for support.

Transportation to appointments:

Your loved one’s discharge plan may outline a number of follow-up tests and appointments that they’ll need you or another family member to transport them to and from.

While a lot of planning and care may be needed, you don’t need to take on all the responsibility.
