Palliative Care Services
We provide the following in-home care support and aged care services for our clients living with a life-limiting illness:
Showering, dressing, toileting and hygiene.
Support with medications
Assist with prompting medications: where a registered nurse or medication-assist trained personal care worker is on shift, medication can be administered.
Meal preparation, cooking, cleaning, laundry, gardening, and pet care.
We can help with a much-needed break or emergency relief for family carers. Overnight care is also available.
Medication and symptom management, care advice and support for you and your loved ones during this time.
Transport to medical appointments or for social outings or shopping.

We provide medication support and support symptom management. This includes supporting pain management or nausea, and around-the-clock care for clients with high-support needs.

Highly experienced and trained in the intricacies of palliative care; assess, plan and administer daily treatment and manage symptoms. The care team will be specifically trained according to the client’s situation, condition and needs.
Shared Care Facilitation
We work alongside your hospital discharge team, GP and community palliative care team to provide seamless care and support, under a shared care model.

Every aspect of our in-home palliative care is tailored to the unique needs of your loved one that will meet their physical, psychological, social, spiritual and cultural needs. Giving you or your loved one the choice to pass away at home.

Start, stop and change care and/or carers, or move to hospital or hospice at any time for any reason. You’re not locked into a set number of hours. You can increase or decrease as needed or even cancel at the last minute if circumstances change.
Supporting Family And Loved Ones
We can support family and loved ones with house work, meal preparation and other domestic duties, allowing you to spend more time with your loved one.

You will be connected with one of our Client Service Coordinators who will look after your rostering requirements and will be responsible for matching your loved one’s care needs with the most suitable nurses and carers.
You will be assigned a Prestige Case Manager who will establish a unique and flexible care program that is personalised to meet your needs. Your Case Manager will check-in with you regularly to ensure you are completely satisfied with both the care and your team.
